UniD is a research project, gathering research data about Audio Description. We are not studying you as an individual or your personal abilities to perform or consume Aduio Description. We are only studying Audio Description as a process and as a product.
This release form does not affect our Privacy Policy or License terms. It's just referring to the public media we make about the project, to share what we are doing with the public (such as in press releases, news stories, or publishing contant about our activities on our website). When you use our UniD systems (website, web tools, mobile apps, etc.) or participate in our research studies, we would appreciate your consent to freely use all of the data we collect (which we will appropriately anonymize). If you would like to restrict that data collection and use in any way, the decision is entirely up to you. We respect and appreciate your privacy and any concerns you might have about its use. That said, our presumption is that you understand this is a research project and that your participation is a key part of us learning about various aspects of Audio Description and sharing what we learn with the public. We need to gather data about the processes you use and the products you make, which means we need your help. We are not gauging your skills or abilities as an individual. We are looking at the big picture, about how people, like you, and including you, make, share, and consume Audio Description.
By participating in our activities, you understand you are participating in The Unidescription Project (an Audio Description research project; IRB Case: 22343).
When involved in this research project, either in person or as a part of an organized event, such as a Descriptathon, I hereby authorize Brett Oppegaard, a researcher at University of Hawaii, (or any member of his research team) to record my name, likeness, image, voice and/or performance on digital media for use in the above described Project / Program. I consent to that media being distributed on the Internet, social media, email, etc., however the research team deems fit on whatever channels they maintain for this purpose. In short, we just want to tell people about what we are doing, and we need your help to tell that story. If you don't want to be a part of that, just let us know.
This media we gather may be edited and otherwise altered at the sole discretion of Dr. Oppegaard and his team and used in whole or in part for any and all broadcasting, audio/visual, and/or exhibition purposes in any manner or media. I understand that I have no rights to the Program or any benefits derived therefrom. This media will be used for educational purposes and in distribution and dissemination of the research to various public communities, including at academic conferences. I also consent to the use of my name, likeness, voice and biographical material about me in connection with the public discourse about the Program. This is just saying that we can select, trim, frame, etc., the material we gather in the ways that work best for the story we are trying to tell about this work.
This is to just say, again, "I understand that my participation is voluntary, and I can stop at any time." You are a volunteer, helping us. We will treat you with respect and do what is right with media about you and your actions and activities related to Audio Description.
This agreement represents the entire understanding of the parties and may not be amended unless mutually agreed to by both parties in writing.
